Payer-compliant goals must be functional, measurable, time-bound, and tied to a real-life activity. Vague goals are the #1 reason for claim denials and audit failures.
The SMART Goal Formula
| Element | What It Means | Example Language |
| Specific | Exactly what skill/behavior | "will produce /r/ in conversational speech" |
| Measurable | Quantified accuracy or frequency | "with 80% accuracy across 3 consecutive sessions" |
| Achievable | Realistic given baseline | Baseline is 40% → goal is 80%, not 100% |
| Relevant | Tied to functional activity | "to improve intelligibility during work meetings" |
| Time-bound | Target date | "within 12 weeks" |
Goal Template
[Patient] will [observable behavior] with [level of accuracy/frequency] given [cue level] in [functional context] as measured by [measurement method] within [timeframe].
Goal Examples by Area
Articulation / Motor Speech
- Patient will produce /s/ blends in spontaneous conversation with 80% accuracy given minimal verbal cues during a 5-minute structured conversation task within 8 weeks.
- Patient will demonstrate intelligible speech in 3–4 word utterances with 75% accuracy to unfamiliar listeners within 12 weeks.
Language (Expressive)
- Patient will use 2-word combinations to request preferred items with 80% accuracy across 3 consecutive sessions within 10 weeks.
- Patient will produce grammatically correct sentences of 5+ words during structured narrative tasks with 70% accuracy within 16 weeks.
Language (Receptive)
- Patient will follow 2-step directions without gestural cues with 80% accuracy across 3 sessions within 8 weeks.
Dysphagia
- Patient will demonstrate safe swallow of IDDSI Level 5 foods with no overt signs of aspiration using compensatory strategies (chin tuck, small bites) with 90% accuracy during structured meal observation within 6 weeks.
Cognitive-Communication
- Patient will use an external memory aid (calendar/phone) to recall daily schedule with 80% accuracy given minimal verbal cues within 12 weeks.
Common Goal Writing Mistakes
- Avoid: "Patient will improve communication skills" — not measurable or functional
- Avoid: "Patient will complete 20 trials of /r/" — describes a task, not a functional outcome
- Avoid: Goals without a baseline — always document where the patient is starting from
- Avoid: Goals that any caregiver could carry out — must require skilled SLP services
